CRITERIA FOR ADMINISTRATIVE PROBATION

Pursuant to the authority in Transportation Code Section 1122, any color scheme permit holder that fails any of thecriteria listed below will be on administrative probation until they correct the problem.  While on administrative probation, a company may not receive any new medallion affiliations or permits during the period of probation.

Any color scheme permit holder which fails to comply with any requirements listed below will be placed on Administrative Probation:

• Maintain updated daily driver assignment roster
• Maintain updated daily vehicle assignment roster
• Information from SFMTA posted in accordance with requirements
• Copy of Workers Compensation information posted in an area visible to drivers
• Gate fee schedules prominently displayed
• Copy of current Business Certificate posted
• Liability Insurance on every vehicle
• Greenhouse gas score of 38 or better
• All required SFMTA permits and permit fees paid
• Meets fax, phone and email requirement
• All vehicles up-to-date with GTU inspections
• Electronic trip information recorded (as of March 1, 2013)
• Staffing during business hours (9am - 5pm)
• Facility available for thorough washing of taxicabs (free of charge)
• Waybills filed and maintained at company's location for six years
• All vehicles have required in-taxi equipment
